Care plans are everywhere in aged care. They’re talked about in meetings, reviewed by staff, updated in systems, and nodded at very seriously.
They’re not a secret document.
They’re not a checklist.
They’re not “done once and filed forever”.
So maybe the real question isn’t whether care plans are person‑centred.
It’s this:
Are we telling people, clearly, often, and in plain language,
that their care plan belongs to them?
Because once people know that, something shifts. Questions get asked. Conversations get better. Care gets more personal.
And honestly, that’s when care plans stop being paperwork and start being useful.
